This user agreement (this "User Agreement") applies to your use of the Patient Portal Account. By signing up for, or otherwise obtaining, a Patient Portal Account, or by accessing or using the Patient Portal Account, you are entering this User Agreement and agreeing to be bound by its terms. For the purpose of this User Agreement, "you" means the individual user of the Patient Portal Account.
