This Host Community Agreement (the “Agreement”) is made as of this day of May 2017 by and between The Patient-Caregiver Connection, Inc., a Massachusetts non- profit corporation with a usual place of business located at 287 Chapman Road, Barre, Massachusetts (“CPC”) and the Town of Barre, Massachusetts, a municipal corporation (“Barre”).
