These Terms and Conditions apply to laboratory testing services that a person obtains from Clinical Reference Laboratory, Inc. (“Laboratory”) through a Marketplace User Agreement (the “Agreement”) that such person (the “Client”) has entered into with FormFox, Inc. (“FormFox”). For purposes of these Terms and Conditions (also referred to herein as these “Terms”), Laboratory and Client may be referred to individually as a “Party” and collectively as the “Parties.” Capitalized terms in these Terms that are not defined in these Terms have the meanings given to them in the Agreement.
