This Contract Extension is made and entered into this 30th day of June, 2022 by and between the Workforce Development Board of Flagler and Volusia Counties, Inc., d.b.a. CareerSource Flagler Volusia (hereinafter CareerSourceFV) a Florida private not for profit corporation, whose principal address is 329 Bill France Blvd. Daytona Beach, Florida 32114, and Eckerd Youth Alternatives, Inc. (hereinafter referred to as Contractor), whose principal addresses are 100 North Starcrest Drive, Clearwater, FL 33765 and extends the original Contract between parties, including any and all Amendments and modifications to the original Contract dated the 30th of June 2020.
