This agreement entered into by and between the Cuyahoga County Board of Health (the "Board"), a political subdivision of the State of Ohio organized under the Constitution and Laws of the State of Ohio, and Lorain County Public Health, (hereinafter referred to as the “Sub- Recipient”) a non-profit organization with its principal office located at 9880 Murray Ridge Rd, Elyria, OH 44035 for and in consideration of the promises, covenants, and conditions hereinafter set forth.
