, (Provider) enters into a Provider Agreement with the State of Alaska, Department of Health & Social Services (DHSS) for the purpose of providing non- medical residential care to an adult experiencing a serious mental illness as defined in 7 AAC 70.990 or an adult experiencing Alzheimer’s or related dementia, who requires the protective oversight of an assisted living home for the State of Alaska’s Alaska Community Living (ACL) Program. By entering into this Provider Agreement, the Provider agrees to the following, including all applicable provisions of the following Appendices:
