AGREEMENT TO PROVIDE SERVICES (FOUNDATIONS COHORT I – YEAR 2)Agreement to Provide Services • June 6th, 2006
Terms: This agreement is between Safe & Civil Schools and the School Board of Clay County. The School Board of Clay County agrees to pay Safe and Civil Schools twenty-five thousand dollars ($25,000.00) in fees for the presentations listed above. In addition, all travel expenses incurred by Susan Isaacs for the presentation will be reimbursed in accordance with School Board policy, not to exceed the amount of $4600. The Jessica Lunsford Act requires a Level II fingerprinting to be on file with the School Board of Clay County before services are rendered. All cost associated with the Level II fingerprinting shall be at no cost to the School Board. An invoice for travel expenses with receipts will be provided by Safe & Civil Schools at the conclusion of each presentation.
