This CivicSmiles Contracting Dentist Agreement (Agreement) is between DeCare Dental Networks, LLC (DeCare) and the dentist or dental entity signing this Agreement (Contracting Dentist). In consideration of participation in this national dental network for publically funded health care programs, I represent and agree as follows:
