This Agreement is made and entered into between the University of Washington School of Nursing (at Seattle, at Bothell and at Tacoma (“School”), located in Washington, and ___________________________________ (“Training Site”), located in _________________, _____________________. The purpose of this Agreement is for Training Site, which is committed to training health care professionals, to provide clinical learning experiences and facilities for School, which desires such placement for its students who are enrolled in its nursing programs (the “education program”).
