I, , hereby authorize the Goodwill Columbus to automatically charge my account on or before the 1st day of the month, for all monthly parking fees, time violation fees and/or damage fees due and payable per the terms of the Monthly Parking Agreement. I understand that the amount of my monthly parking fee is subject to change by Goodwill Columbus per the terms of the Monthly Parking Agreement, or due to fees or other charges, as specified in the Monthly Parking Agreement (e.g. late fees).
