This Qualified Service Organization Agreement (this “QSOA”) is made as of Current Date (the “Effective Date”) by and between the Healthix, Inc. (“Service Organization”), a New York not- for-profit corporation with its principal office at 40 Worth Street, New York, NY 10013 and Provider Name (“Treatment Provider”), with its principal office at Provider Address, New York, NY 10035 collectively referred to as the “Parties.”
