(this “Agreement”) is entered into by and between INDIANA HEALTH INFORMATION EXCHANGE, INC. (“IHIE”), and the entity whose name appears on the signature page of this Agreement (“Tippecanoe County Health Department”) and is effective as of the date upon which IHIE executes this Agreement (the “Effective Date”). IHIE and Tippecanoe County Health Department are each referred to as a “party” and together as the “parties” in this Agreement.
