I, (print model name) , agree to perform as a model for Chuckles, Inc. and its authorized distributors in hair shows, performances and/or photographs. In this role, I agree not to commit any act, or do anything, that might hurt Chuckles’ business reputation or otherwise embarrass the company. The products used on my hair will be Chuckles, Inc. products.
