Congratulations to you and your child for participating in the LAUSD Instrumental Music Program. Your child has been selected to borrow an LAUSD instrument. Please read the instrument loan agreement below. Sign and return the form to your child’s music teacher immediately. The instrument will not be released to the student until the signed form is received by the teacher. Please make sure you have completed all the information on the form below. Students will not receive their instrument if the form is incomplete.
