This agreement is entered into on between the City of Springfield’s Office of Housing, which is the HMIS Lead for the Hampden County Continuum of Care and is hereafter known as “Office of Housing”, and (agency name), hereafter known as "Agency," regarding access, use and sharing of data with the Hampden County Continuum of Care Homeless Management Information System, hereafter known as "Hampden County HMIS."
