AGREEMENT BETWEEN THE HEALTH CARE PROVIDER ANDVolunteer Health Care Provider • April 18th, 2010
THIS CONTRACT is entered into between the State of Florida, Department of Health, hereinafter referred to as the "department", and hereinafter referred to as the "health care provider", for the purpose of improving access to health care for indigent residents by providing governmental protection to health care providers who offer free, quality health care services to underserved populations of the state. This contract is to ensure that health care professionals who provide such services as agents of the state are provided sovereign immunity while acting within the scope of duties pursuant to this contract and the requirements of the applicable health care practitioner laws and administrative rules.
