THIS VIEW ONLY ACCESS AGREEMENT (“Agreement”), by and among South Shore Health System, Inc., a Massachusetts not-for-profit corporation (“South Shore”/“SSHS”), comprising as of the date of this Agreement the following affiliates: South Shore Hospital, Inc., Coastal Medical Associates, Inc. d/b/a South Shore Physician Ambulatory Enterprise, Health Provider Services Organization of the South Shore, Inc., and South Shore Medical Center, Inc.; and [INSERT FULL CORPORATE NAME OF PROVIDER ENTITY] which has a primary business address of [ADDRESS] (“Provider”) (South Shore and Provider are collectively the “Parties”; each is a “Party”), is effective as of [INSERT DATE­ WRITE OUT IN FULL] (the “Effective Date”).
