This Agreement (“Agreement”), effective as of July 1, 2018 (“Effective Date”) is entered into by and between Managed Access to Child Health, Inc. d/b/a Partnership for Child Health (“THE PARTNERSHIP”), a Florida not-for profit organization located at 910 North Jefferson Street, Jacksonville, Florida 32209 and CHILDREN’S HOME SOCIETY OF FLORIDA (“CHS”). The purpose of this Agreement is to conduct work related to Empowered Children Empowering Communities, an America's Promise Alliance—Healthy Schools project.
