As you recall, your Board approved an independent contractor agreement between the Human Resources Agency (HRA) and Case Management, Inc. on August 23, 2005, to provide expert services involving program development, contract monitoring, and case management and documentation for the Multipurpose Senior Services Program. Because the services of this contractor are still necessary and allocated funds remain unspent, HRA seeks to extend the ending date of this agreement. The purpose of this letter is to request approval of an amendment to change the original ending date of this agreement from December 31,2005 to June 30,2006.
