This commitment to raise funds through a sponsored activity and agreement to the terms and conditions overleaf must be signed by anyone undertaking a sponsored activity on behalf of Lindsey Lodge Hospice. By signing this document you agree to undertake the activity stated below, on the date indicated and agree to raise funds for Lindsey Lodge Hospice, Registered Charity No.702871and pay all money raised by you to Lindsey Lodge Hospice within 60 days of the stated activity. Please be aware that failure by you to pay funds raised to Lindsey Lodge or return funds to sponsors is theft and may result in legal action against you.
