This Wisconsin Medicaid Program Requirements Attachment to the Wisconsin Vision Services Agreement (the “Agreement”) is entered into by and between Compcare Health Services Insurance Corporation dba Anthem Blue Cross and Blue Shield (“HMO”) and March Vision Care Group, Incorporated (“Subcontractor”) and is incorporated into the Agreement. Subcontractor agrees that the provisions set forth in Exhibit H apply to services provided to Members who participate in the Wisconsin BadgerCare Plus and SSI programs (“Wisconsin Medicaid Program”). To the extent any provisions herein conflict with the provisions set forth in the Agreement, the provisions in Exhibit H shall control for those services provided to Wisconsin Medicaid Program Members. Subcontractor shall include the provisions set forth in Exhibit H in each Participating Provider agreement and if not expressly included, such provisions shall be deemed to be incorporated by reference.
