This Organization Access Agreement ("Agreement") is entered into by and between Availity, L.L.C. ("Availity", "We", "Us", or "Our") and the Organization ("Organization", "You", or "Your" means a Medical Provider or Provider group that provides medical goods and services to an individual) named herein, a "Covered Entity" as defined in 45 CFR §160.103. Availity and You are sometimes hereinafter referred to individually as the "Party" or collectively as the "Parties".

The following is an Agreement for Medical Providers to access the Availity secure web site. The Primary Controlling Authority must sign the Agreement. The Agreement is then submitted to Availity at the Fax number: 904.470.4770. The information will be reviewed and verified for completeness. If there are any problems with the information provided, Availity will attempt to resolve them to expedite the approval process. When the Agreement is complete, Availity will notify the Organization via an e-mail to the PAA indicating that access to the Availity secure web site is now available.
