This Grant Contract, by and between the State of Tennessee, Department of Health, hereinafter referred to as the “State” and Contractor Legal Entity Name, hereinafter referred to as the “Grantee,” is for the provision of Ryan White Part B Core Medical and Support Services and Minority AIDS Initiatives (MAI), as further defined in the "SCOPE OF SERVICES."
