I, , the SUBSCRIBER set forth in the “Elite Software, Inc. License and Monthly Subscription Agreement” (the “Agreement”) entered into by me on with Elite Software (“ELITE”), hereby request ELITE cancel said Agreement. I understand the effective date of cancellation will be the date this Cancellation Request and the Security Device (as defined in the Agreement) are both received by ELITE. I also understand I will not receive a refund of any previous payments made by me to ELITE. I have recorded the RMA number received from ELITE at the top of this page on the right.
