The above named child has been enrolled as a student of Barrie School, a Non-profit Corporation (hereafter, referred to as “The School”) for the 2019-2020 academic year, through a separate agreement. I/We hereby request that the optional extended-day service indicated below be provided by The School for the above named child. I/We agree to pay the total sum indicated to The School for the service selected below. All amounts not paid when due are subject to late charges. Payment Plans: Check the appropriate box below for the service desired. The payment plan selected on the Direct Debit Form will be used for all payments due to The School. (When two or more students are enrolled in the extended-day program, each contract will receive a ten percent (10%) reduction.)
