This Agreement is entered into by and between TEXAS TECH UNIVERSITY HEALTH SCIENCES CENTER (hereinafter referred to as "TTUHSC"), a public institution of higher education in the State of Texas, on behalf of its School of Medicine Graduate Medical Education Programs, and [LEGAL NAME AND LEGAL STATUS DESCRIPTION OF OTHER PARTY] (hereinafter referred to as "FACILITY"). Both TTUHSC and FACILITY are herein referred to as "Party" or, collectively, "Parties."
