This Independent Provider Agreement (“Agreement”) is made and entered into this 1st day of July, 2022, by and between Glenn County, a political subdivision of the State of California (“County”) through its Health and Human Services Agency’s Behavioral Health Division ("Behavioral Health"), and Casa Serenity, LLC, (“Provider”).
