This Agreement (hereinafter referred to as “Agreement”) is entered into between the State of Connecticut (hereinafter “STATE”) acting through the Office of Health Strategy (hereinafter “OHS”) pursuant to Connecticut General Statutes § 19a-754a, and Connecticut Public Health Association, Inc., a company having its principal offices at 83 Oak Street, Hartford, CT 06106 (hereinafter “CONTRACTOR”). The parties agree that the services specified below shall be provided by CONTRACTOR in strict compliance with the provisions of this Agreement.
