WHEREAS, the purpose of this AFFILIATION AGREEMENT is to guide and direct the parties respecting their affiliation, working arrangements and agreements in furtherance thereof to provide high-quality learning experiences for clinical students in the Texas Woman’s University System Programs and/or Departments listed in Exhibit B, hereinafter referred to as “Program”. This AFFILIATION AGREEMENT (the “AGREEMENT”) is made between TEXAS WOMAN’S UNIVERSITY SYSTEM, a public institution located in Texas, hereinafter referred to as “TWU,” COORDINATING ENTITY, an entity physically located in [STATE], hereinafter referred to as “COORDINATING ENTITY,” and HOST FACILITY, an entity physically located in [STATE], hereinafter referred to as “HOST FACILITY.” TWU, COORDINATING ENTITY and HOST FACILITY shall each be a “Party” and shall be collectively referred to hereinafter as the “Parties.”
