THIS AGREEMENT is between North Sound Behavioral Health Administrative Services Organization ("North Sound BH-ASO") and the undersigned facility provider Valley Cities Counseling and Consultation (hereinafter referred to as the "Facility Participating Provider"). This Agreement will become effective upon the date set forth in North Sound BH-ASO’s executed Acceptance Letter (the "Effective Date"). This Agreement sets forth the terms and conditions under which Facility Participating Provider shall participate in one or more networks developed by North Sound BH-ASO as a Facility Participating Provider of Psychiatric Inpatient Services in the North Sound Regional Service Area, which includes Island, San Juan, Skagit, Snohomish and Whatcom Counties.
