THIS MEMBERSHIP AGREEMENT (the “Agreement”) is effective as of this of , 20 (the “Effective Date”) by and between OSFPGA Foundation (“OSFPGA”), a United States not-for-profit organization, and (“Member”), hereinafter individually and collectively referred to as the “Party/Parties“. Members of the OSFPGA Foundation shall be referred to collectively as the “Members”.
