This policy is entered into by and between THE BOARD OF COMMISSIONERS OF TIPPECANOE COUNTY ("Employer") and Anthem Insurance Companies, Inc. d.b.a. Anthem Blue Cross and Blue Shield ("Anthem") for the purpose of establishing stop loss coverage and is effective as of 01/01/2022 upon the terms and conditions herein ("Policy").
