This Amendment is hereby entered into by and between Genoa Healthcare, LLC, 3140 Neil Armstrong Blvd, Ste. 110, Eagan, MN 55121 (hereinafter “Contractor”) and Lancaster County, (hereinafter “County”), for the purpose of amending the Contract dated May 22, 2018, under County Contract No. C-18-0230 for Annual Services Pharmacy Services for General Assistance Clients, RFP No. 18-025, which is made a part hereof by this reference.
