I, (Participant) acknowledge that I am freely entering into an agreement, to practice certain yoga and fitness classes and participate in certain programs, workshops and other yoga and fitness related activities (the Services), with Wabi-Sabi Yoga and Wellness Center, LLC (Agreement). I represent and warrant that I am in good physical health and that I do not suffer from any health or medical conditions which would limit my participation in the Services. I understand that it is solely my responsibility to advise an instructor of any limitations that I may have prior to engaging in the Services. I understand that it is my responsibility to consult with a licensed health professional prior to and regarding my participation in any of the Services. I understand the risks associated with the Services and I agree to follow all instructions so that I may safely participate in the Services.
