THIS AGREEMENT is made and entered into as of the 1st day of January, 2023 by and between Employee Assistance Associates and Consultants of Oklahoma, an Oklahoma Professional Limited Liability Company, with offices at 6585 South Yale Avenue, Suite 340, Tulsa, Oklahoma, 74136, hereinafter referred to as “EA ASSOCIATES”, and City of Coweta, a municipal corporation, with offices at 310 South Broadway, Coweta, Oklahoma 74429, hereinafter referred to as “CITY”.
