This evaluation agreement (this “Agreement”) is made as of the latter of the dates of signature at the end of this document (the “Effective Date”), between RaySearch Laboratories AB (publ) (“RaySearch”), and the [NAME OF CLINIC], or purchasing organization representing the Clinic, as the case may be (“Clinic”), each referred to herein individually as a “Party” and collectively as the “Parties”.
