This Class Action Settlement Agreement and Release (the “Agreement” or “Settlement”) is made by and between Plaintiffs Nathan Hofstader (“Hofstader”) and Richard Cerenzia (“Cerenzia”) (or jointly as “Plaintiffs”), individually and on behalf of the class they seek to represent, on the one hand, and Defendants Providence Health and Services Washington (“PHS-WA”) (named as Providence Holy Family Hospital and, Sacred Heart Medical Center,1 (“Sacred Heart”)), and Providence Health and Services (“PHS”) (or jointly as “Defendants”), on the other.
