This Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement (“Release”) is made and provided by the person signing below (“Participant”), who desires to participate in “Cushing Cycle Challenge” fitness and fundraising activities (the “CCC”) promoted, conducted by or associated with the Cardinal Cushing Centers (“CUSHING”) and its employees or agents. Participant acknowledges and agrees that entering into this Release is required as a condition to participating in the CCC and accessing any virtual or other materials related thereto. This Release shall be effective on the date of its execution and delivery by Participant.
