THIS AGREEMENT, made and entered as of the date of on-line connection, by and between Pine Telephone Company, Inc. dba, Pine, Pine Cellular Phones, Pine-net, Broken Bow TV Cable, Pine Rural Cable ("PINE"), and the Internet access subscriber, ("Customer"), upon the following terms and conditions ("Agreement"), including any attachments and documents incorporated herein by reference.
