(the “Work”) to be published by the Casualty Actuarial Society (“CAS”) in its (E-Forum). Based on good and valuable consideration, the Copyright Holder gives permission to the CAS to reproduce, reprint, distribute, and sell the final edited version of the Work, and to place the Work on the CAS Web Sites. The Copyright Holder authorizes the CAS to use and to publish the name, photograph, and biographical data of the author(s) in connection with the use and promotion of Work. [If the original and/or primary authors are not the Copyright Holder, the names of such authors are , and Copyright Holder also authorizes the CAS to publish the name, photograph, and biographical data of such author(s) in connection with the use and promotion of the Work.]
