THIS AGREEMENT is made and executed in duplicate at Los Angeles, California, as of June 24, 2021 by and between CHILDREN’S HOSPITAL LOS ANGELES, a non-profit organization which owns and operates a pediatric patient care and teaching center, (hereinafter referred to as "Hospital" or “CHLA”), and FULL NAME, M.D. (hereinafter referred to as "Physician in Training") (collectively, the Hospital and the Physician in Training are referred to as “parties”).
