This Agreement is made and entered into as of JANUARY 1, 2023 (the “Effective Date”) by and between Janus Associates, Inc. d.b.a. BHS (hereafter known as “BHS”) and Park Hill School District (hereafter known as “Customer”) and outlines the terms under which BHS shall provide Employee Assistance Program (EAP) and workplace services to Customer. A participant is defined as all employees and household members (hereafter known as “Participants”).
