Waiver: For and in consideration of permitting (child's name) to enroll and participate in Bee Research Activities of the Nieh Research Lab at UCSD given by THE REGENTS OF THE UNIVERSITY OF CALIFORNIA, in the City of San Diego, County of San Diego and State of California, beginning on the 8th day of the month of September, 2011, and ending on the 11th day in the month of October, 201, hereinafter called “The Activity”, I, for myself, my heirs, personal representatives, or assigns, do hereby release, discharge, waive and covenant not to sue THE REGENTS OF THE UNIVERSITY OF CALIFORNIA, its officers, employees and agents from liability from any and all claims including the negligence of The Regents of the University of California, its officers, employees, and agents, resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation in The Activity.
