This Mutual Agreement to Arbitrate Claims (this “Agreement”) dated as of April 1, 2015 is made by and between Mathnasium, LLC, a California limited liability company, and its subsidiaries and affiliated entities (the “Company”) and (“Staff Member”) in consideration of the mutual promises contained within this Agreement. Any reference in this Agreement to “I” or “me” or “my” shall mean the Staff Member identified above and any reference in this Agreement to the “Company” shall mean Mathnasium, LLC, and all of its subsidiaries and affiliated entities. The Company and Staff Member are sometimes referred to in this Agreement individually as a “Party” and collectively as the “Parties.”
