Contract Type FiledMarch 3rd, 2017, (Provider) enters into a Provider Agreement with the State of Alaska, Department of Health & Social Services (DHSS) Division of Behavioral Health (DBH)for the purpose of providing community-based services to Children and Youth (21 and under) experiencing Severe Emotional Disturbance (SED), and/or Substance Use Disorder (SUD), who are at risk for out- of-home placement, residential or institutional care; in need of supports to transition to a lower level of care; or need assistance stabilizing during an emergent or ongoing crisis. By entering into this Provider Agreement, the Provider agrees to the following, including all applicable provisions of the Appendices listed herein.
