I, , hereby authorize Covenant United Methodist Church and/or its affiliated associations to use without recourse my child’s image for promotion and/or publicity in print, video or on the web site and including Facebook. For good and valuable consideration, I hereby irrevocably consent to and authorize Covenant United Methodist Church and/or its affiliated associations to use and reproduce any and all photographs/images taken for any purpose whatsoever, without the necessity of any compensation being paid to me. I further agree to hold Covenant United Methodist Church and/or its affiliated associations harmless from any and all claims that may arise from the use of said photographs/images. All negatives and positives, together with any prints, are the property of Covenant United Methodist Church and/or its affiliated associations, solely and completely.
