Dated: In order to prevent the owner (the “Seller”) of the professional practice from losing referrals, patients, or employees, and to keep the practice intact for the professional who takes over this practice, the following is agreed: is hereinafter referred to as Prospective Buyer / Associate, who in addition to the items below grants permission to Diamontina Ochoa, hereinafter referred to as Broker/Consultant, to forward emails to Buyer/Associate regarding prospective opportunities. This Agreement shall be enforced in accordance with the laws of the State of California Prospective Buyer/Associate wishes to evaluate confidential information regarding practice opportunities presented byBroker/Consultant: • This confidentiality extends to all documentation presented on the practice, all phone conversations regarding the practice, and all financial and ancillary information, including the fact that the practice is for sale.• Prospective Buyer/Associate recognizes that any confidential
