These Standard Terms and Conditions (these “Terms and Conditions”) form a part of the Modernizing Medicine Podiatry Systems, Inc. Order Form (the “Order Form” and together with these Terms and Conditions, this “Agreement”) between Modernizing Medicine Podiatry Systems, Inc., a Delaware corporation with offices located at 4850 T-Rex Avenue, Suite 200, Boca Raton, Florida 33431 (“Company”), and the customer set forth on the Order Form (“Client”).
