CELEBRITY GAME & LIL’ DUNKERS BBALL CLINIC PARTICIPANT CONTRACT The 3rd Annual Clifton R Lewis Good Life Foundation Celebrity Basketball Game and Dunk Contest will be held on Saturday April 27, 2013 at Greater Nanticoke Area HS 425 Kosciuszko St. Nanticoke, PA 18634, starting at 6:30 PM. We ask for you to make your travel plans and arrive at the designated hotel by Friday night 4/26/13. By signing this participant contract, you acknowledge that your travel expenses and/or hotel accommodations will be paid in advance by our foundation. If you need to cancel your participation in our event, we require a cancellation notice at least 60 days prior to event. If no cancellation notice is received via email to clewis@crlgoodlife.org you are responsible to repay any of your own travel fee’s/reservations.
