This Sublease Agreement (the "Sublease") is made effective as of insert date here, by and between Jennifer Sweeton, owner of Kansas City Mental Health Associates (Trade Name for Mind Works Consulting and Psychological Services, PLLC; "Tenant"), and insert name here ("Subtenant"). Tenant has previously entered into a lease agreement with Richard B. Summers Trust and Jacqueline M. Summers Trust ("Landlord") dated April 01, 2017 (the "Prime Lease"). This sublease agreement incorporates and is subject to the original lease agreement between the Landlord and their Tenant, a copy of which is attached hereto, and which is hereby referred to and incorporated as if it were set out here at length. The Subtenant agrees to assume all of the obligations and responsibilities of the Tenant under the original lease for the duration of the sublease agreement.
